1) Member Name and Address

2) Membership Purchase Date

2) MBSconnect Username and Password.

3) Last four (4) digits of the credit card used to purchase your membership.

Once your refund request is reviewed, we will send you a refund status update. Please know it can take up to 30 days for your refund to post to your credit card.
